Job Title: Registered Nurse Case Manager

A Path of Care Home Health is seeking a skilled Registered Nurse Case Manager to join our team. As a RN Case Manager, you will play a critical role in the provision and coordination of care for our home health care agency.

About the Role

As a Registered Nurse Case Manager, you will be responsible for providing direct patient care to acute and chronically ill patients in the comfort of their homes. You will validate patient assessments through explicit documentation and assist patients in managing their own time and learning how to adapt to new situations.

You will also use your critical thinking skills to ensure patient safety needs are met and they always feel comfortable around you. Your tasks will fluctuate based on your specific client caseload and schedule.

Responsibilities
  • Provide direct patient care to acute and chronically ill patients in their homes
  • Validate patient assessments through explicit documentation
  • Assist patients in managing their own time and learning how to adapt to new situations
  • Use critical thinking skills to ensure patient safety needs are met
Requirements
  • Current RN license
  • Emotional intelligence and the ability to show compassion and empathy toward others
  • Ability to overcome challenges and defuse conflict
Work Schedule

This nursing position works 8 AM - 5 PM, Monday to Friday.
