Job Summary

We are seeking a highly skilled Registered Nurse Case Manager to join our team. As a key member of our care team, you will be responsible for coordinating patient care, developing care plans, and providing direct patient care to acute and chronically ill patients in the comfort of their own homes.

You will work closely with our interdisciplinary team to ensure that patients receive the best possible care, and that their needs are met in a timely and effective manner. Your expertise and compassion will make a real difference in the lives of our patients and their families.

Responsibilities
  • Develop and implement individualized care plans for patients
  • Coordinate patient care with our interdisciplinary team
  • Provide direct patient care to acute and chronically ill patients in their homes
  • Validate patient assessments through explicit documentation
  • Assist patients in maintaining their personal autonomy and adapting to new situations
  • Use critical thinking skills to ensure patient safety needs are met
Requirements
  • Current RN license
  • Emotional intelligence and ability to show compassion and empathy
  • Ability to overcome challenges and defuse conflict
