Job Title: RN Case Manager

About the Role:

We are seeking a skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will be responsible for providing high-quality patient care, coordinating services, and collaborating with interdisciplinary teams to ensure the best possible outcomes for our patients.

Responsibilities:
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide education and support to patients and families to promote self-care and independence.
  • Coordinate services with other healthcare providers and community resources to ensure seamless care.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improvement.
  • Document patient information and progress in a timely and accurate manner.
Requirements:
  • Graduate of an accredited school of nursing with a current RN license.
  • One to two years of recent acute care experience in an institutional setting and one to two years of recent experience in home care.
  • ADN/Bachelor's degree with one year of hospice care experience preferred.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
