Terminal Care Support Specialist

About the Job: As a Terminal Care Support Specialist at ProHealth Home Health & Hospice, you will play a vital role in providing end-of-life care to patients and their families. Your primary responsibility will be to deliver compassionate and dignified care, while promoting the well-being of those around you.
Key Responsibilities:
- Provide Personalized Care: Deliver individualized care to patients, taking into account their unique needs, preferences, and goals.
- Support Families: Offer emotional support and guidance to families, empowering them to make informed decisions about their loved one's care.
- Maintain Professional Development: Stay up-to-date with industry developments and best practices, ensuring you provide the highest quality care possible.
- Collaborate with Healthcare Professionals: Work effectively with nurses, doctors, and other healthcare professionals to coordinate care and achieve optimal outcomes.
- Document Patient Information: Accurately record patient information, including medical history, treatment plans, and progress notes.
Requirements:
- Education: High school diploma or equivalent
- Training/Licensure: Completion of an approved training and competency course
- Experience: Minimum 2 years of experience as a nursing assistant, preferred
- Active Patient Interaction: Active patient interaction within the past three years, preferred
Working Conditions: This role requires considerable physical exertion throughout the day, including kneeling, squatting, reaching, twisting, climbing, walking, and exposure to temperature and humidity changes. You must also be able to assist in lifting and/or transferring patients weighing up to 100 pounds.
Compensation and Benefits: We offer a competitive salary of $48,000 - $60,000 per annum, depending on experience. Additionally, we provide comprehensive benefits, including health insurance, paid time off, and opportunities for professional growth and development.
