We are seeking a highly skilled Medical Social Associate to join our team at SIU Medicine. As a Medical Social Associate, you will play a critical role in providing medical case management services to our patients.
Key Responsibilities- Case Management: Conduct client intake interviews to determine eligibility for our program, develop care plans, and provide referrals to medical and support services.
- Client Education and Support: Provide education and support to clients regarding their disease, including HIV/AIDS.
- Program Administration: Establish and maintain working relationships with service providers, act as a resource person to healthcare professionals, and input client information into our database.
- Teaching and Research: Participate in local, regional, and national meetings, provide educational information to clients and clinic patients, and cooperate with physician faculty projects.
- Bachelor's or Master's degree in social or behavioral science or related health specialty.
- One year of progressively more responsible full-time experience in social services or medical/clinical setting.
