Medical Care Coordinator

GeorgiaGov is seeking a highly skilled and dedicated Nurse Specialist to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support to individuals with complex medical needs.
Key Responsibilities- Conduct comprehensive assessments of individuals' health care needs, including medical, social, and emotional background.
- Develop and implement individualized care plans, taking into account physicians' treatment plans and individuals' level of need and functional ability.
- Provide health education instruction to consumers on various health issues, including treatments, self-care, and surgical procedures.
- Assess individuals' health problems and needs, develop and implement nursing care plans, and maintain accurate and well-organized medical records.
- Communicate effectively with agency personnel, service providers, support coordinators, and other individuals involved in the care and service provided to consumers.
- Bachelor's degree in nursing from an accredited college or university OR Associate's degree in nursing from an accredited college or university AND current Georgia license as a Registered Professional Nurse.
- Any related professional work experience.
- Vacation: 15 days each year up to 5 years of service (accrual at a rate of 10 hours per month), 18 days each year from 5-10 years of service (accrual at a rate of 12 hours per month), 21 days each year for 10 + years of service (accrual at a rate of 14 hours per month).
- Sick Leave: 15 days each year (accrual at a rate of 10 hours per month).
- Holidays: The state of Georgia recognizes 12 paid holidays.
