RN Hospice Care Specialist

Job Summary:
As a Weekend On-Call Hospice RN with Agape Care Group, you will be a vital part of our patient care team, providing high-quality care and support to patients facing life-limiting illnesses and their families.
Key Responsibilities:
- Advocate for patients and ensure they receive the best possible care
- Collaborate with the care team to develop and implement patient care plans
- Provide emotional support and guidance to patients and their families
- Participate in ongoing education and training to stay up-to-date on best practices in hospice care
Requirements:
- Graduate of an accredited school of nursing with a current state license as a registered nurse
- 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
- Ability to sit, stand, bend, move intermittently and lift at least 25lbs
