Key Responsibilities:
  • Provide individualized support to residents, addressing barriers to progress and promoting goal achievement.
  • Maintain accurate records of resident participation and progress, ensuring timely reporting and documentation.
  • Develop and implement effective case management strategies, linking residents to off-site services and programs as needed.
  • Collaborate with community partners, service providers, and property management to ensure seamless service delivery.
  • Plan and coordinate health and wellness events, community building activities, and daily adult education groups/workshops.
  • Input data into the Homeless Management Information System (HMIS) accurately and on a timely basis.
Requirements:
  • Over 18 years of age.
  • Experience working with individuals experiencing chronic homelessness and mental health challenges.
  • Familiarity with Housing First, Harm Reduction, and/or Recovery Oriented service models.
  • Ability to maintain relationships with residents while maintaining appropriate boundaries.
  • Ability to interact in a culturally diverse community.
  • AA degree preferred; or equivalent experience serving homeless individuals.
  • Valid California driver's license and availability of an insured vehicle.
Work Environment:

As a Supportive Services Coordinator, you will work with a diverse population, including individuals experiencing homelessness, mental health challenges, and poverty. You will be exposed to various environments, including shelters, recreational rooms, and common areas. The noise level will vary depending on the activity.

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Must be able to lift 25 lbs and travel by automobile 10%.
