Responsibilities
  • Coordinate patient care services, including scheduling appointments and managing care plans
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless coordination of care
  • Monitor patient outcomes and make adjustments to care plans as needed
Requirements
  • RN license without disciplinary actions
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
