Patient Care Coordinator

We are seeking a skilled RN Care Manager/Resident Care Manager to join our team at the Life Care Center of Port Townsend in Washington. As a key member of our care management team, you will be responsible for coordinating patient care and services, promoting quality care through collaboration and coordination with patients, families, care teams, payers, and external entities.
Key Responsibilities:
- Promote quality care by collaborating with patients, families, care teams, payers, and external entities.
- Carefully coordinate patient care and services to meet individual needs.
- Develop and implement effective care plans that address patient goals and outcomes.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions of care.
- Monitor and evaluate patient progress, making adjustments as needed to ensure optimal care.
Requirements:
- Nursing diploma (associate's or bachelor's degree in nursing)
- Currently licensed/registered in applicable State
- One (1) year of clinical experience in post acute care setting preferred
- Prior case management, utilization review, and discharge planning experience preferred
- Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) preferred
