Clinical Care Navigator

We are seeking a dedicated and experienced RN to join our team as a Clinical Care Navigator at Life Care Center of Port Townsend, Washington. This is a full-time day shift position with opportunities to cover on an as-needed basis for med cart/floor duties. A competitive sign-on bonus of up to $7,500 is available for this role.
Company OverviewLIFE CARE CENTER OF PORT TOWNSEND offers a peaceful setting overlooking the Kah Tai Lagoon and featuring stunning views of the Olympic Mountains. Our facility provides inpatient and outpatient rehabilitation, skilled nursing care, and short-term and long-term care services.
About the RoleThe Clinical Care Navigator (RN) plays a critical role in coordinating patient care and services. They will promote quality care by working collaboratively with patients, families, caregivers, payers, and external entities to ensure seamless transitions from our facility to other care settings. The ideal candidate will possess strong communication skills, be able to generate clear and concise written communication, and demonstrate expert knowledge in their field.
Key Responsibilities- Coordinate patient care and services to promote quality outcomes
- Serve as liaison between patients, families, caregivers, payers, and external entities
- Implement standards of practice for care management and coordination of care
- Train and educate patients, families, associates, and other providers of care
- Nursing diploma (associate's or bachelor's degree in nursing)
- Current licensure/registration in applicable State; maintain active Registered Nurse (RN) license in good standing throughout employment
- One year of clinical experience in post-acute care setting preferred
- Prior case management, utilization review, and discharge planning experience preferred
We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience. Additionally, you will have access to our comprehensive benefits package, which includes health, dental, and vision insurance, 401(k) matching, and paid time off.
