We are seeking a skilled and compassionate LPN to join our Primary Care team in Battle Ground, Washington. As a key member of our healthcare team, you will provide high-quality patient care and support to our clinicians and patients.
Key Responsibilities- Provide nursing and administrative support to clinicians and other healthcare team members
- Conduct focused/limited nursing assessments and triage on routine or self-limiting medical conditions
- Recognize signs and symptoms of common and routine medical conditions/illnesses and implement planned nursing and medical interventions as delegated by RN/clinician
- Prepare patients for actual visit/procedure prior to clinician's exam during rooming-in process
- Assure exam/procedure room contains appropriate supplies, equipment, forms, and educational materials are available and ready for use during visit/procedure
- Provide health teaching/education or advice on routine or self-limiting medical conditions, procedures, tests, surgeries, self-injections, etc.
- Utilize standardized tools to determine patient's level of engagement in self-care as directed by RN
- Use KP HealthConnect and other information systems to document all pertinent data and outcomes of nursing care according to standardized procedures and policies
- Graduate of accredited school of nursing
- Practical Nurse License (Washington) within 6 months of hire OR Compact License: Practical Nurse within 6 months of hire
- Practical Nurse License (Oregon) within 6 months of hire
- Basic Life Support
- Two (2+) years of nursing experience in assigned departments clinical area (e.g. Pediatrics, FP/IM, OB/Gyn, Surgery, Allergy, Dermatology, Urology, etc.) within an ambulatory care setting
- IV therapy experience or recent certification
- Familiarity in documenting in an electronic medical record system
