Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Battleground of Cascadia, LLC. As an LPN, you will provide high-quality patient care and work collaboratively with our healthcare team to ensure the best possible outcomes for our residents.

Key Responsibilities
  • Provide direct patient care and assist with care as directed by the RN, consistent with your scope of practice and competency.
  • Initiate the SBAR process and collaborate with the RN for assessment and recommendations.
  • Make rounds with physicians and other team members, as needed.
  • Participate in patient evaluations and care planning activities, reviewing and revising patient assessments and care plans as needed.
  • Coordinate patient care activities and communicate changes to other team members and patient family/significant other.
  • Oversee Medication Aide/Tech in medication/treatment administration.
  • Administer, transcribe, communicate, and implement physician orders, obtaining RN signature as required by state regulations.
  • Communicate with patient's physician.
  • Assist with emergencies; administer cardiopulmonary care performed and observations of patient status in the clinical record promptly by utilizing standard clinical documentation guidelines as required by the company, and local, state and federal rules and regulations.
Requirements
  • Graduate of accredited LPN School of Nursing.
  • Current valid LPN license in the state employed.
  • Valid CPR certification.
  • Preferred: 6 months experience in a long-term care environment.

Please note that all employees of Cascadia Healthcare are required to submit and be cleared to work in the facility per each state's specific background check requirements prior to contact with patients/residents.
