At Interim HealthCare of the Twin Cities, we are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality home care services to patients in the Southwest Twin Cities Metro area.

Job Summary

We are looking for a dedicated and experienced LPN to work in a home care setting, providing one-on-one care to patients in their own homes. As an LPN with our team, you will have the opportunity to work independently, making a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Participate in ongoing interdisciplinary assessment of the patient
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s)
  • Provide services ordered by the physician as indicated in the plan of care
  • Recognize and report changes in the patient's condition to the RN
  • Counsel and educate the patient and family in meeting healthcare-related needs
Requirements
  • Current license as an LPN in the State of MN
  • 2 years' experience as an LPN
  • Able to travel within the geographic area serviced by the office
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • CPR Certification
