As a Home Care Licensed Practical Nurse (LPN), you'll work independently in a patient's home environment, providing skilled nursing services and promoting their health and well-being.
Key Responsibilities:- Participate in ongoing interdisciplinary assessment of the patient's needs and develop a plan of care in partnership with the patient, representative, and caregiver(s).
- Provide services ordered by the physician, including medication administration, wound care, and vital sign monitoring.
- Counsel and educate patients and families on meeting healthcare-related needs and promoting independence.
- Recognize and report changes in the patient's condition to the RN and collaborate with the interdisciplinary team to ensure seamless care.
- Competitive hourly rate: $29.51 - $35.88/hour
- $6,000 hiring bonus

- Current license as an LPN in the State of MN, active and in good standing
- 2 years' experience as an LPN
- Able to travel within the geographic area serviced by the office, with a valid driver's license, reliable transportation, and proof of auto insurance
- CPR certification
