Salary Information

The estimated salary for this position is $90,000 - $110,000 per year, depending on qualifications and location.

Job Description

We are seeking a skilled RN Clinical Care Coordinator to join our Case Management Team. This role provides clinical case management services to enhance patient-centered care and maximize outcomes across the patient care continuum.

Key Responsibilities:
  • Cordinate case management process from patient entry to post-discharge, including outpatient settings.
  • Identify patients suitable for case management intervention based on cost, case complexity, and other criteria.
  • Monitor and document quality of care to ensure patient care plan goals are appropriate and implemented.

The ideal candidate will possess strong critical thinking, communication, and problem-solving skills, with a focus on delivering high-quality patient care and promoting effective resource utilization.

Required Skills and Qualifications

To be successful in this role, you must have:

  • A current license to practice as a Registered Nurse in the State of Utah.
  • Two years of professional experience in a clinically related area.

Prior experience in clinical resource management activities and third-party payer interactions is preferred.
