Job Description:
The Child and Family Specialist provides skill-building, advocacy, and behavioral support to clients enrolled in our Wraparound program. Responsibilities include assessing client needs, preparing individualized plans of care, and providing targeted interventions with family, child, and community agencies.
Key Responsibilities:
- Perform 1:1 rehabilitative skill-building for clients with significant psychosocial impairment or physical health problems.
- Review medical, psychosocial, school, and other needs to develop comprehensive plans of care.
- Meet with clients, families, treatment staff, placement staff, consultants, and other agency personnel to understand and meet client needs.
- Develop written Individual Plans of Care and Crisis Response Plans under supervision.
- Intervene in emergency crises to provide support and assistance in problem resolution.
- Provide skills-building interventions to improve client behavioral outcomes.
- Mobilize resources to meet client needs, including consultation with social security, welfare, and health departments.
