About the Role

We are seeking a highly skilled Licensed Clinical Social Worker to join our team as an Early Childhood Development Expert. This role will play a critical part in our mission to improve the health and happiness of the communities we serve.

Key Responsibilities
  • Conduct team-based well-child visits to monitor development, social-emotional functioning, and relational health, including providing anticipatory guidance.
  • Consult with pediatric patients/parents about behavioral aspects of their medical treatment following the Healthy Steps tiered service delivery.
  • Evaluate patients to identify social, emotional, and economic factors which may interfere with obtaining maximum benefits from medical care.
  • Develop and implement a social work treatment plan to be offered to patients/parents.
  • Provide brief psychotherapy to patients and families.
  • Respond promptly and with caring actions to patients/parents and employees.
  • Maintain the child development support line, including responding and tracking call requests within the designated response time.
  • Deliver and assess Modified Checklist for Autism in Toddlers (M-CHAT), Patient Health Questionnaire (PHQ9), and the Ages and Stages Questionnaires (ASQ2/3) tools.
  • Document and share patient screening scores and barriers with primary care and/or behavioral health providers, including appropriate consult documentation is secured in patient medical records.
  • Acknowledge and honor patients' psychosocial, spiritual, and cultural beliefs.
Provider & Team-Based Care
  • Build strong relationships with families and providers to increase the efficiency of the medical system and support team-based comprehensive care.
  • Collaborate with Healthy Steps implementation team to implement core components of the program.
  • Maintain open communication with members of the primary care team.
  • Work with pediatric primary providers around care coordination, goal setting, coaching, and education about key aspects of a child's development.
  • Provide consultation and/or facilitates training to medical professionals and practice staff regarding early childhood development, early relational health/infant early childhood mental health, and trauma-informed practice.
  • Utilize evidence-based approaches when engaging with patients to increase their sense of control over their health, such as motivational interviewing to promote collaboration between patients and their medical/behavioral care team.
  • Engage in continuous quality improvement (CQI) initiatives to ensure the Healthy Steps model is delivered with fidelity.
  • Maintain professional working relationships with all levels of staff, clients, and the public.
  • Participate on the Healthy Steps implementation team, including attending regular team meetings.
  • Tracks caseload to ensure capacity to deliver the Healthy Steps services within the risk stratified service delivery model, including ensuring exit criteria is met and families/children are elevated to higher levels of care or referred to community providers, as needed.
Clinical Administration
  • Applies Healthy Steps tiers of service delivery when providing services to children and their families in the Healthy Steps program.
  • Designs, markets, conducts, and evaluates health promotion programs for child development, social-emotional, and behavioral screenings for family needs, such as maternal depression, risk factors, and social determinants of health.
  • Participates in case conferences, departmental or agency meetings, and trainings, as directed.
  • Participates in care coordination and collaboration with Interfaith Community Services shared patients.
Requirements
  • Master's degree in social work required.
  • Valid CA Clinical Social Worker License (LCSW) required.
  • Two years' psychotherapy experience (pre or post doc/licensure) working directly with children, adults, or adolescents required.
  • Experience providing health psychology services with an emphasis in early childhood development and screening assessments required.
  • Bilingual (Spanish/English) required.
