Job Summary

We are seeking a skilled Occupational Therapist to join our team at St. David's Healthcare at Home. As a Home Health Occupational Therapist, you will play a vital role in providing high-quality, patient-centered care to our patients in the comfort of their own homes.

Responsibilities
  • Conduct comprehensive patient assessments to identify therapeutic, rehabilitative, and functional needs.
  • Develop and implement individualized care plans in collaboration with the patient care team and physician.
  • Provide purposeful and skilled interaction care to support patients throughout their home health journey.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Prioritize caseload, determine daily staffing needs, and coordinate assignments/handoffs to ensure efficient care delivery.
Requirements
  • Bachelor of Science in Occupational Therapy from an accredited program.
  • Successful completion of the National Certification Examination for Registered Occupational Therapist.
  • State license in practicing state.
  • Reliable transportation and proof of valid automobile liability insurance.
  • Must have valid driver's license.
