Position Overview

Key Responsibilities:

  • Collaborate with clinicians to evaluate, strategize, and coordinate patient care, aiming to fulfill health requirements and elevate patient outcomes.
  • Support the Quality Director in maintaining high standards of clinical practice.

Essential Qualifications:

  • Educational Background: A Bachelor's degree in a relevant clinical field is mandatory. Candidates should possess three to five years of progressive clinical experience, including a minimum of one year in home healthcare. Alternatively, candidates with RN, PT, OT, or SLP credentials and over five years of clinical experience, including home care, are also eligible.
  • Licensure/Certifications: Must hold a current industry-recognized OASIS certification or be willing to obtain it within six months of employment.
  • Experience: Refer to the educational requirements.
  • Must have licensure in the appropriate clinical field.
  • Possess a valid driver's license and have access to a vehicle for work-related tasks, covered by liability insurance as per agency policy.
  • Current certification in Basic Life Support for Healthcare providers is required.
  • Demonstrated proficiency in OASIS and home health coding is crucial.
  • Exemplary communication and facilitation skills, with the ability to identify and manage conflicts effectively.
  • In-depth knowledge of homecare reimbursement methodologies, including Medicare Prospective Payment System (PPS) and emerging pay-for-performance strategies is essential.
