St. Luke's Health System is seeking a skilled Nurse Care Coordinator to join our Home Health team. As a key member of our team, you will be responsible for providing high-quality patient care and coordinating services to ensure seamless transitions between care settings.
Key Responsibilities:
- Provide individualized care to patients in their homes, focusing on their unique needs and goals.
- Collaborate with multidisciplinary teams, including physicians, nurses, and other healthcare professionals, to develop and implement comprehensive care plans.
- Coordinate services, such as home health aides, physical therapy, and occupational therapy, to ensure patients receive the necessary support.
- Monitor patient progress, identifying areas for improvement and making adjustments to care plans as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure clear understanding of care plans and progress.
Requirements:
- Current RN license in the state of practice.
- 2 years of RN experience, with a focus on home health or acute care.
- Basic Life Support (BLS) certification.
- Valid driver's license and proof of auto insurance.
