Role Description

The Licensed Practical Nurse (LPN) is a vital member of the healthcare team, holding a valid license from the New York State Department of Education. Candidates should ideally possess a minimum of two years of experience in supporting individuals with intellectual disabilities. The LPN operates under the guidance of a Registered Nurse, delivering essential nursing care to clients as needed, including direct support when required. Responsibilities include reviewing staff documentation related to various health metrics and taking necessary actions based on findings.

Key Responsibilities
  • Document nursing notes as necessary, detailing significant medical changes, interventions, recommendations, and communications with healthcare providers, day services, and families.
  • Maintain accurate medical records in compliance with OPWDD standards and best practices.
  • Continuously monitor clients' medical needs and health status, provide necessary treatments, document medical histories, and implement protective measures against communicable diseases.
  • Ensure timely filling and refilling of prescriptions, accurately record physician's medication orders, oversee medication administration, report any medication errors, and dispose of outdated or discontinued medications according to established protocols.
  • Review clinical data and upload relevant medical information into the health management system.
  • Update health information records and add healthcare providers as needed.
  • Compile and present weekly reports to the interdisciplinary team.
Qualifications
  • Associate degree in Nursing with a minimum of two years of professional experience in healthcare delivery to individuals with Intellectual Developmental Disabilities.
  • Current licensure as a Licensed Practical Nurse is mandatory.
Skills and Abilities
  • Strong presentation and written communication skills.
  • Ability to translate organizational philosophy into actionable practices and represent the organization professionally.
  • Proficient in Electronic Health Records.
  • Possession of a valid Driver's License for travel to various locations.
