Job Summary:
The Geriatric Care Provider - Senior Medical Care Specialist is a critical member of our interdisciplinary team, responsible for providing primary care to our patients in their homes. This role requires a strong commitment to delivering high-quality, patient-centered care, while also ensuring the coordination and transition of care across the healthcare continuum. As a key member of our team, the successful candidate will be accountable for engaging new patients, completing accurate and timely documentation, and collaborating with our broader care team to ensure seamless care delivery.
Key Responsibilities:
- Provide Primary Care in Home-Based Settings: Deliver high-quality, patient-centered care to older adults in their homes, addressing their unique medical, social, and emotional needs.
- Collaborate with Interdisciplinary Team: Work closely with our broader care team, including physicians, nurse practitioners, social workers, and other healthcare professionals to ensure seamless care delivery and coordination.
- Initial Patient Screenings and Assessments: Conduct initial patient screenings and assessments, creating appropriate assessment and care plans, and completing timely and accurate documentation.
- Care Coordination and Transition: Ensure the coordination and transition of care across the healthcare continuum, working closely with our care team to ensure seamless care delivery.
- Medication Management: Prescribe and manage medications, including psychopharmacological agents, as appropriate, while adhering to state supervisory agreements.
Requirements:
- Minimum Master's Degree in Nurse Practitioner Studies: From an accredited, master's-level NP program.
- Active, Unencumbered RN and NP Licenses: In the posted state, with current board certification, either AANC or AANP.
- BLS Certification and Active NPI#: As an NP, with CPR certification.
- Active, Unencumbered DEA Licensure: Or eligible to apply for upon hire.
- Previous Experience in Home Care, Primary Care, Hospice, Palliative Care, or Geriatrics: Strongly preferred, with other complex patient healthcare experience also beneficial.
- Valid Driver's License and Car Insurance: With access to a car and willingness to drive to patient locations.
Salary Range: $110,000-$137,000 per year, based on experience.
Travel Requirements: Up to 100% locally to deliver patient care.
Work Conditions: Ability to lift up to 30 lbs, with occasional lifting of up to 50 lbs, and standing for extended periods.
