Job Summary
We are seeking a compassionate and skilled Senior Care Professional to join our team. In this role, you will be responsible for developing and implementing individualized care plans that meet the unique needs of our residents. You will work closely with our interdisciplinary team to identify and address the social and emotional needs of our residents, ensuring that they receive the highest level of care possible.
Key Responsibilities
- Develop and implement individualized care plans that meet the unique needs of our residents.
- Collaborate with our interdisciplinary team to identify and address the social and emotional needs of our residents.
- Provide direct care to our residents, including assistance with daily living activities and medication management.
- Participate in the planning and implementation of resident care, including developing and implementing care plans and providing input on resident needs.
- Maintain accurate and up-to-date records of resident care, including progress notes and care plans.
Requirements
To be successful in this role, you will need:
- A high school diploma or equivalent.
- At least 1 year of experience in a healthcare setting, preferably in a long-term care facility.
- Strong communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare teams.
- Able to lift, bend, and move residents with care and compassion.
- Ability to work a flex schedule, including evenings, weekends, and holidays.
- Current certification in CPR and First Aid.
Salary and Benefits
