Job Summary:

The UNC Health Care organization is seeking a skilled and compassionate Nurse Case Manager to join our team. As a key member of our Care Management department, you will play a vital role in providing high-quality patient care and support.

Key Responsibilities:

  • Conduct comprehensive assessments of patient needs and develop personalized care plans.
  • Collaborate with healthcare teams to ensure seamless care coordination and effective communication.
  • Provide education and support to patients and their families to promote health and well-being.
  • Monitor patient progress and make adjustments to care plans as needed.

Requirements:

  • Graduate from an accredited school of nursing, BSN preferred.
  • Two (2) years of healthcare experience as a Registered Nurse.
  • Licensed to practice as a Registered Nurse in the state of North Carolina.

Work Environment:

This is a full-time position in a hospital setting, with a standard work schedule of 40 hours per week. The ideal candidate will be highly organized, adaptable, and able to work effectively in a fast-paced environment.
