About the Job

As a Senior Care Transition Specialist, you will be responsible for coordinating care transitions, collaborating with healthcare professionals, and empowering residents to take control of their health. You will work closely with our team to develop and implement individualized care plans, ensuring that each resident receives the highest level of care possible.

Key Responsibilities:
  • C Coordinate care transitions for residents, ensuring a smooth transition from hospital to home or other care settings.
  • D Develop and implement individualized care plans, taking into account the unique needs and goals of each resident.
  • E Collaborate with healthcare professionals, including physicians, nurses, and therapists, to ensure comprehensive care.
  • F Empower residents to take control of their health by providing education and support.
Requirements:
  • R Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing required.
  • Bachelor of Science in Nursing preferred.
  • M Minimum of 2 years of related nursing experience, preferably in senior services.
  • S Minimum of 3 years in a supervisory capacity required.
