Empath Health Hospice RN Case Manager

We are seeking an experienced and compassionate RN Case Manager to join our Central Facilities Team. As a key member of our hospice care team, you will play a vital role in providing high-quality patient care and support to patients and their families.

Key Responsibilities:

  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Coordinate patient care with the interdisciplinary team, including physicians, nurses, and other healthcare professionals
  • Provide education and support to patients, families, and caregivers on disease management, palliative care, and end-of-life care
  • Collaborate with the healthcare team to ensure seamless transitions of care
  • Document patient progress and outcomes in a timely and accurate manner

Requirements:

  • Graduate of an approved nursing program and current Florida RN license
  • Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a hospice RN preferred
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
