Core Responsibilities:

  • Facilitates case management by collecting essential screening and intake documents necessary for assessing eligibility and aiding Veteran Participants in accessing VA and community resources.
  • Distributes outreach materials to Veteran clients and community organizations.
  • Safeguards client and partner agency documentation while maintaining confidentiality in compliance with applicable laws and guidelines.
  • Engages with veterans and their families through walk-in hours and scheduled appointments to address eligibility and service inquiries.
  • Assists veterans in compiling required documentation for applications, including proof of veteran status, income verification, housing status, and identification for family members.
  • Conducts follow-ups with veterans post-intake to assist in gathering any missing documentation and to gauge their ongoing interest in completing the application process.
  • Performs initial assessments of veterans' situations to identify potential needs and directions once enrolled, such as substance abuse history or mental health concerns.
  • Informs veterans who are not eligible or enrolled about the reasons for their status, the appeal process, and connects them with additional resources for support.

Qualifications:

  • Minimum education: High School Diploma or equivalent.
  • Experience in a social service setting, with knowledge of veteran services preferred.
  • Strong communication and organizational abilities.
  • Capability to manage multiple tasks in a customer service-oriented environment.
  • Proficiency in Microsoft Office Suite (Excel, Outlook, Word).
