Job Summary

ConcertoCare is seeking a highly skilled Practice Manager to serve as the operations leader and manage the performance of an interdisciplinary team within a value-based care framework. The ideal candidate will have a strong background in healthcare administration, operations management, and team leadership.

Key Responsibilities
  • Data-Driven Management: Oversee team performance to ensure high-quality care delivery, manage day-to-day market operations, and personnel management of medical assistants, community health workers, and care navigators.
  • Operational Support: Manage team schedules, oversee clinical and operational task completion, and handle patient complaints and grievances.
  • Clinical Support: Monitor urgent situations, support the team in addressing urgent care needs, and manage care transition episodes as needed.
  • Personnel Management: Hire, onboard, train, and manage medical assistants, community health workers, and care navigators, including ongoing performance management and career development.
  • Team Support: Provide hands-on shadowing, collaboration, guidance, and support to the team.
  • Workflow Management: Monitor clinical operational workflows, ensure adherence to standard procedures, and identify opportunities to improve workflow design and efficiency.
  • Process Implementation: Lead the implementation of new processes, programs, and pilots, and serve as a market super-user and trainer for systems and initiatives.
  • Meetings and Training: Develop and lead meetings required for management, training, education, and team building.
  • Compliance: Ensure team adherence to compliance policies, procedures, and standards of conduct, including applicable laws and regulations.
Requirements
  • Education: Associate degree required, Bachelor's or Master's degree preferred, with a healthcare-related specialization.
  • Experience: 3+ years of management/operations experience at care delivery organizations, health systems, or provider groups, with experience in value-based care, home health, and multi-site operations a plus.
  • Leadership: Experience working with interdisciplinary teams, with 3+ direct reports, and previous P&L ownership a plus.
  • Skills: Proficiency in data analysis, knowledge of applicable regulations and state and federal laws, and ability to travel as needed.
Compensation

Base Salary/Wage Range: $90,000 to $105,000 plus annual bonus. Compensation is commensurate with qualifications, skills, competencies, and experience.
