Job Description

Reliance Home Health Caregivers is seeking a skilled Home Health Care Manager to join our team. As a key member of our staff, you will be responsible for overseeing the delivery of high-quality care to our clients. Your primary duties will include:

  • Evaluating worker performance and providing feedback to ensure that performance standards are met.
  • Determining and interpreting work procedures to promote efficient service delivery and ensure client needs are met.
  • Communicating with diagnostic, professional, and developmental training/education staff to ensure concerted action between units.
  • Examining and evaluating primary and collateral information to monitor case records and ensure adherence to standards.
  • Conducting regularly scheduled staff meetings to coordinate service delivery to clients.
  • Training new staff members or conducting in-services for current staff using necessary materials in accordance with Individual Advocacy Groups policies and procedures.
  • Participating in meetings of an informational or problem-solving nature to establish and maintain effective communication and perform necessary organizational business.
  • Writing and developing sequence of steps or procedures to achieve specified results or goals, adapting and modifying plans as changes in situations and/or client require.
  • Writing, reviewing, and evaluating individual program plans, referring to recommendations of both clients and client's previous performance and outcomes, making adjustments and changes as needed.
  • Monitoring and writing incident reports using staff and client information, case records, and knowledge of the situation, recommending and approving plans for resolution.
  • Reinforcing proper behavior, language, and social patterns to improve or increase consistency of independent functioning of the client.
  • Exchanging information about clients or client programs with diagnostic staff, colleagues, or supervisors in verbal or written format as required to maintain records, plan, or implement services.
  • Formulating client objectives and evaluating client performance on assigned objectives, examining outcome against stated criteria, using rating scales, tracking sheets, monthly progress notes, etc., to ensure personal growth and improvement in specific skills.
  • Inspecting and monitoring clients for appropriateness, noting clothing, hygiene, behavior, etc., in reference to standards and taking corrective action if required to maintain the dignity, health, welfare, and safety of clients.
  • Substituting for absent coworkers in the performance of assigned duties with direction from supervisors to ensure the day-to-day functioning of the unit.
  • Completing required IPP reports, forms, progress notes, etc., and using performance records, tracing sheets, and established guidelines to maintain compliance with agency and state requirements.
  • Evaluating and discussing worker understanding of policies, job behavior, etc., in individual conferences to inform workers of results of work performance.

Requirements:

Education/Experience:

  • Four-year college degree and 1 year minimum of directly related work experience.

Skills/Knowledge:

  • Follows methods and procedures in delivery of instructional, training, and support services.
  • Knowledge of and implementation of each designated individual's needs, goals, interventions, and supports.
  • Completion of required records keeping, reports, and assignments.
  • Quality of interactions with supervisors, unit staff, consumers, and outside individuals.
  • Efficient/cost-effective use of time, materials, and human resources.
  • Quality of coordination of work and monitoring of work performance.
  • Proficient in word processing, database management, and spreadsheet, e.g., Word Access or Excel.
  • Must possess strong interpersonal skills.
  • Must possess a valid driver's license.
  • Must possess current vehicle insurance.
  • Must be over 25 to operate an agency vehicle.
