Certified Nursing Assistant Long Term Care Specialist

Our sub-acute floor is a 59-bed Medicare unit that provides care to residents with multiple clinical issues, including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac-related issues. Our team works closely with healthcare professionals, including doctors, social workers, dietitians, PT/OT, hospice, and spiritual support, to provide a well-rounded opportunity for learning, professional growth, and personal satisfaction.
Our Chronic Care FloorOur 60-bed Medicare/Rehabilitation and long-term care unit is a great place to learn basic med-surg, I.V. therapy, and wound treatments nursing skills. Our team works with a variety of healthcare professionals to provide and implement the best treatment plan for residents. This fast-paced and busy unit provides a great atmosphere for PT/OT residents to be motivated and reach their goals.
Our Dementia Care FloorWe provide care to residents with a variety of diagnoses on our 60-bed Medicare/Medicaid/Dementia unit. Our team creates a fullness of life through the joy of relationships, the art of caring, and the spirit of living. We work closely with a multidisciplinary team to discuss and implement the resident's plan of care.
About the FacilityBeaumont Commons - Farmington Hills is a 3-floor facility with 9 units, providing care for long-term care residents and post-acute rehab patients. Our first floor has 59 beds with a focus on sub-acute rehab, the second floor has 59 beds focusing on long-term care and sub-acute rehab, and our third floor is a secured dementia unit. We care for dementia residents, long-term care, and sub-acute rehab residents, with a variety of programs for residents to participate in multiple activities.
