Certified Nursing Assistant Long Term Care Specialist

Our sub-acute floor is a 59-bed Medicare unit that provides care to residents with multiple clinical issues, including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac-related issues requiring monitoring.
Nurses work closely with a variety of healthcare professionals, including doctors, social workers, dietitians, PT/OT, hospice, and spiritual support teams.
Our sub-acute unit offers a well-rounded opportunity for learning, professional growth, and personal satisfaction.
Our Chronic Care floor is a 60-bed Medicare/Rehabilitation and long-term care unit, providing a great place to learn basic med-surg, I.V. therapy, and wound treatments nursing skills.
Nurses on this unit work with a host of healthcare professionals to provide and implement the best treatment plan for residents.
This fast-paced and busy unit provides a great atmosphere for PT/OT residents to be motivated and reach their goals.
About the FacilityBeaumont Commons – Farmington Hills is a 3-floor facility with 9 units, providing care for long-term care residents and post-acute rehab patients.
The first floor has 59 beds with a focus on sub-acute rehab, the second floor has 59 beds focusing on long-term care and sub-acute rehab, and our third floor is a secured dementia unit.
Nurse-patient ratios vary depending on the shift and the population being served.
We care for dementia residents, long-term care, and sub-acute rehab residents.
We accept patients with various specialties, such as general rehab, neuro rehab, cardiac, respiratory, and cancer.
We offer various programs for residents to participate in multiple activities.
Our goal is to create a Fullness of Life through the joy of relationships, the art of caring, and the spirit of living.
Scope of WorkAs a team member, you will act as a team member in the delivery of quality, direct patient care.
You will perform basic skills per department guidelines and communicate observations to members of the healthcare team.
You will provide direct patient care under the direction of the Unit Manager or Licensed Nurse, consistent with policies and procedures of the unit.
You will be responsible for direct day-to-day care of assigned residents/patients, including dressing, bathing, transporting, and feeding.
You will document care provided as directed in a manner consistent with guidelines.
