Primary Care Physician
Job Summary:Uptown Community Health Center, Inc is seeking a Primary Care Physician to provide comprehensive, coordinated primary and preventive healthcare services to an ethnically diverse patient population.
Key Responsibilities:- Oversee Resident Physicians
- Evaluate and treat patients within the licensed scope of practice; appropriately refer patients for specialty evaluation and treatment.
- Provide preventive health advice and case management and provide consultation to other staff.
- Provide or arrange for comprehensive primary care services in a primary medical care ambulatory clinic setting, including laboratory and diagnostic workup and specialty referrals.
- Participate in multi-disciplinary, team-based care of patients.
- Responsible for accurate, timely, and complete documentation in patient records in accordance with Uptown Community Health Center policy.
- Rend care to UCHC patients in accordance with quality and standards as established by UCHC, the standards of care prevailing in the community, applicable law, and according to state and federal grant requirements for non-discrimination regardless of status, race, sex, religion, national origin, handicap, age, or ability to pay.
- Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and the need for follow-up as appropriate during each patient visit. Verify and document that the patient understands the diagnosis, treatment, and follow-up recommendations.
- Serve as a role model for effective staff teamwork and actively maintain positive morale and attitude.
- Valid license to practice the profession in the applicable state.
- Board certification/eligibility based on education and specialty.
- Valid DEA in the applicable state.
- MD or DO degree from a fully accredited medical or osteopathic school of medicine.
- Residency training in the area of specialty.
- Experience with Epic application and health record.
- Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals.
- Bilingual English / Spanish, complete comprehension, literacy, and fluency in both languages.
