LPN Supervisor for Personal Care

We are seeking a skilled LPN Supervisor to join our team at Garden Spot Communities, a nonprofit organization dedicated to providing compassionate personal care to our residents.
Job Summary:
The LPN Supervisor will be responsible for assisting residents with daily living activities, performing daily rounds to ensure policies are being followed, and ensuring accurate documentation of medical records.
Responsibilities:
- Assist residents with daily living activities, including personal laundry and reminders of activities
- Perform daily rounds on Meadow View units to ensure policies are being followed
- Assist with resident admissions, transfers, discharges, and deaths
- Ensure residents receive medication as prescribed by a physician
- Ensure accurate documentation of medical records and reporting forms
- Oversee medication changes and medication passes to ensure staff compliance
Requirements:
- Licensed Practical Nurse with current Pennsylvania State License
- Minimum of two years of nursing experience, supervision, and clinical experience or education in areas such as restorative nursing, dementia care, personal care, or long-term care
- Current CPR certification
- Basic computer skills
