LPN Care Coordinator

We are looking for a dedicated and caring Nursing Care Specialist to join our team at Brethren Care Village. This is an exciting opportunity to work in a dynamic and supportive environment where you can make a real difference in the lives of our residents.
Salary:The salary for this position is $24-$25.80 per hour, with a night shift differential and weekend shift differential.
Job Description:As a member of our Assisted Living or Skilled Nursing team, you will be responsible for providing high-quality nursing care to our residents. This includes assessing their needs, developing and implementing individualized care plans, and collaborating with other healthcare professionals to ensure seamless care.
- Develop and implement care plans that address resident's physical, emotional, and social needs.
- Provide direct patient care, including wound care, medication management, and vital sign monitoring.
- Collaborate with interdisciplinary teams to coordinate resident care and services.
To succeed in this role, you will need:
- An LPN license in good standing in Ohio
- CPR certification or willingness to obtain upon hire
- Experience working in a long-term care setting
- Excellent communication and interpersonal skills
