Job Summary

We are seeking a highly skilled Registered Nurse to join our team at the State of Washington. As a Medical Recuperative Care Nurse, you will play a critical role in providing care for vulnerable and high-risk Veteran patients at our Transitional Housing Program.

Key Responsibilities
  • Review discharge paperwork to ensure accurate treatment plans, medication lists, and diagnosis information.
  • Collaborate with the VA Liaison and DVA MRC case manager to develop and implement comprehensive care plans.
  • Participate in monthly care reviews and ad-hoc case review meetings to ensure optimal patient outcomes.
  • Report significant changes in client status, medication intake, or overall condition to the VA Liaison and DVA MRC case manager.
  • Modify care plans as needed in collaboration with the VA Liaison and DVA MRC case manager.
  • Submit required VA documentation to the VA Liaison.
  • Review discharge documentation to ensure complete understanding of MRC client medication needs.
  • Provide education to MRC clients on medication types and administration.
  • Complete and maintain medication-monitoring documentation.
  • Measure and record vitals for MRC clients based on care plans.
  • Assess clients to determine if emergency medical interventions are needed.
Requirements
  • Current Washington State registered nurse license or unencumbered registered nurse multistate license.
  • One year of experience facilitating discharge plans and continuous care-planning activities.
Preferred Qualifications
  • Experience working with Veteran populations.
  • Experience working with individuals experiencing homelessness.
