Job Summary

As a Licensed Practical Nurse with Avamere Communities, you will play a vital role in providing high-quality patient care and coordinating services to ensure the best possible outcomes for our residents.

Responsibilities

Coordinate with nursing staff and support personnel to plan daily services, programs, and activities that meet the unique needs of each resident.
Prepare and administer medications, reviewing medication cards for accuracy and completeness.
Review resident care plans to ensure goals, problems, approaches, and revisions are aligned with nursing needs.
Provide leadership to nursing personnel and make daily rounds to ensure staff is providing direct care in accordance with resident care plans and wishes.
Participate in facility surveys and maintain accurate records of resident care information in accordance with HIPAA guidelines.

Qualifications

Possess a nursing degree from an accredited college or university.
Hold a current, unencumbered, active license to practice as an LPN in this state.
Demonstrate knowledge of nursing and medical practices, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
Experience with Electronic Medical Records and computer documentation systems.
Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
