Job Summary

As a Nurse Care Specialist with Duke HomeCare & Hospice, you will provide professional nursing care to patients in their homes or alternate-site settings. Your primary focus will be on delivering high-quality, patient-centered care that meets the unique needs of each individual.

Key Responsibilities
  • Plan, provide, and document professional nursing care for patients in accordance with the NC nurse practice act, physician orders, and established policies and procedures.
  • Use comprehensive assessment skills to plan priorities and set realistic outcomes, collaborating with the healthcare team to evaluate and cost-effectively deliver patient care.
  • Assess educational needs of patients and families, developing and implementing effective discharge plans to ensure a smooth transition to post-acute care.
  • Role model professional nursing through conduct, appearance, communication, mutual respect, ethical decision-making, critical thinking, and problem-solving skills.
  • Participate in weekly interdisciplinary team meetings and case conferences to ensure appropriate care and services are provided to patients.
Requirements
  • Current North Carolina RN licensure or compact RN licensure in a participating state.
  • Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) from the American Heart Association.
  • Two years of med-surgical, critical care, or home health/hospice/infusion experience.
  • Valid driver's license and current automobile insurance coverage.
  • Prior experience in Home Health, Hospice, or Infusion is preferred.
Preferred Qualifications
  • Associate Degree or Diploma in Nursing.
  • Registered Nurses are encouraged to enroll in a DUHS-approved BSN program after completing two years of service.
Work Environment and Requirements

Perform patient care primarily in the patient's residence or skilled nursing facility (more than 50%). Travel to and from patient residences is required.
