Role Summary:

The California Advancing and Innovating Medi-Cal (CalAIM) initiative is a transformative multi-year program led by the Department of Health Care Services (DHCS) aimed at overhauling healthcare access for all Californians. In this role, you will be instrumental in guiding Medi-Cal members through the healthcare landscape, ensuring they receive comprehensive care. You will be at the forefront of fostering a more coordinated, person-centered, and equitable health system that extends beyond conventional clinical environments to address all aspects of physical and mental health.

This position is non-clinical in nature.

Key Responsibilities:

Your primary focus will be to assist individuals, families, and youth experiencing homelessness in securing affordable and permanent housing solutions. You will be responsible for establishing a housing program under Abrazar CalAIM and positioning Abrazar as a key housing access point.

  • Facilitate the establishment of the agency as a housing access point within the Orange County Coordinated Entry System (CES).
  • Develop and implement a housing program as part of Abrazar CalAIM Community Support services.
  • Support members in their housing search and negotiations.
  • Conduct outreach efforts to raise awareness of the housing program and available services.
  • Evaluate member eligibility for the housing program and assess their housing needs.
  • Assist members in locating available properties and negotiating lease terms.
  • Help members address landlord-related issues.
  • Provide ongoing support and resources to ensure housing stability.
  • Connect members with additional services and resources as required.
  • Collaborate with government entities, community organizations, housing authorities, and property owners.
  • Educate members about their tenant rights and responsibilities.
  • Maintain accurate and organized documentation of member interactions and services provided.
  • Stay informed about community resources and services.
  • Engage members effectively while upholding program standards during in-person and virtual meetings.

CalAIM Care Manager Support:

Your secondary responsibility will involve assisting Medi-Cal members, particularly those experiencing homelessness with complex healthcare needs, in navigating the healthcare system. This includes addressing access barriers, building relationships with service providers, coordinating comprehensive care, and monitoring health interventions and outcomes.

  • Deliver necessary direct services and referrals as a member of the CalAIM team, primarily focusing on individuals and families facing homelessness.
  • Conduct initial and ongoing assessments of members to identify their needs, including medical, mental health, substance use, financial, housing, and other support services.
  • Develop individualized care plans with input from providers and members, adhering to CalAIM standards.
  • Collaborate with healthcare professionals to create and implement care plans for members with chronic health conditions.
  • Enhance communication and relationships between community partners and service providers.

Qualifications:

We seek candidates who:

  • Possess experience in housing navigation and familiarity with Housing CES in Orange County.
  • Have knowledge of chronic medical conditions, mental health issues, substance use, and homelessness, along with a basic understanding of public benefits.
  • Exhibit strong case management and documentation skills, including the ability to conduct needs assessments and develop action plans.
  • Can manage confidential information in compliance with HIPAA standards.
  • Are self-motivated and capable of managing multiple projects in a fast-paced environment.
  • Adapt readily to dynamic situations and changes.
  • Demonstrate a strong commitment to the mission and objectives of Abrazar.
  • Are flexible with work hours, including evenings and weekends as necessary.

PREFERRED EXPERIENCE:

  • Extensive housing navigation experience in a housing specialist role within Orange County.
  • Experience assisting clients with benefit applications and navigating healthcare systems.
  • Training in relevant areas such as Motivational Interviewing, Trauma-Informed Care, and Crisis Intervention.
  • Proven expertise in community outreach and engagement.
  • 1+ years of case management experience or a similar role with data management systems.
  • Proficiency in Microsoft Office Suite, particularly Excel.
  • Ability to work independently and collaboratively within a team.
  • Familiarity with available programs and services in Orange County.
  • Strong interpersonal, organizational, and communication skills.

MINIMUM REQUIREMENTS:

  • Bachelor's Degree in a relevant field.
  • Experience in housing navigation or a related role, preferably in Orange County.
  • One year of experience in case management with vulnerable populations or in healthcare and homeless services.
  • Proficiency with technology and data management systems.
  • Bilingual in Spanish or Vietnamese is preferred.
  • Availability to work occasional Saturdays.

MANDATORY CRITERIA:

  • Verification of employment eligibility.
  • Willingness to obtain CPR/First Aid certification.
  • Successful completion of a Tuberculosis Test, Background Screening, and Drug Test as a condition of employment.

DRIVING REQUIREMENTS:
Driving is a critical function of this role. Candidates must:

  • Have reliable transportation.
  • Possess a valid driver's license.
  • Provide proof of insurance and ownership for personal vehicles used for work.
  • Qualify for Abrazar's insurance coverage.

PHYSICAL AND ENVIRONMENTAL DEMANDS:

  • Ability to stand or sit for extended periods during meetings and community events.
  • Capability to function effectively in an office environment and perform daily tasks.
  • Ability to communicate clearly and accurately.
  • Capacity to transport materials to and from meetings.
  • Ability to drive to various locations for member meetings.

  • Occasional lifting of materials over 50 pounds.

Benefits:

  • Starting hourly wage of $23-25.
  • Paid Time Off - Vacation (12 days annually), accrued monthly after 3 months.
