Job Summary

We're seeking a highly skilled and compassionate Registered Nurse to join our team at One Medical Seniors. As a Nurse, you'll work closely with our physicians, health coaches, and other clinical team members to provide comprehensive, coordinated care to adults 65 and older. Your primary responsibilities will include:

  • Providing direct patient assessment and care
  • Phone and walk-in triage for patients
  • Oversight of anti-coagulation clinic activities and patient monitoring
  • Transitions of care coordination
  • Participating in clinical development of Health Coaches
  • Participating in process and quality improvement initiatives
Requirements

To be successful in this role, you'll need:

  • 2+ years of nursing experience
  • Critical thinking and judgment in the provision of clinical care
  • Compassionate, kind, and open-minded approach to patient care
