Nurse Navigator Job Description

We are seeking a highly skilled RN Nurse Navigator to join our team at Trinity Health Of New England. This critical role involves coordinating all aspects of care for patients undergoing complex orthopedic procedures, from pre-surgery planning to post-operative recovery.

Key Responsibilities
  • Develop and implement comprehensive nursing care plans in collaboration with the multidisciplinary team
  • Provide evidence-based patient education and support throughout the patient journey
  • Ensure seamless transitions of care between hospital and home settings
Requirements
  • Graduate of an accredited School of Nursing (BSN required)
  • Current licensure as a registered nurse (RN)
  • CPR certification
  • Three to five years of nursing experience, with surgical patient care preferred

The estimated annual salary for this position is $90,000 - $115,000, depending on experience. We offer a comprehensive benefits package, including medical, dental, and vision coverage, 401(k) matching, and paid time off.
