Job Summary

We are seeking a pediatric social worker to join our team who is passionate about helping patients and families navigate life-limiting illnesses. As a pediatric hospice social worker, you will be responsible for providing psychosocial evaluations, ongoing counsel, bereavement services, and community education outreach. You will work in accordance with the plan of care to provide physical, emotional, and spiritual support to patients and families when it's needed most.

Key Responsibilities
  • Conduct psychosocial evaluations to assess patient and family needs
  • Provide ongoing counsel and support to patients and families
  • Develop and implement bereavement services to support patients and families
  • Participate in community education outreach to promote awareness and understanding of pediatric hospice care
Requirements
  • MS degree in social work from an accredited school of social work
  • Current state license as a social worker
  • 2+ years of clinical work experience, preferably in healthcare or hospice
  • Reliable transportation and ability to lift at least 25lbs
