Job SummaryWe're seeking a pediatric social worker to join our team who is committed to creating meaningful experiences for patients and their families. As a pediatric hospice social worker, you will be responsible for psychosocial evaluations, ongoing counsel, bereavement services, and community education outreach. Working in accordance with the plan of care, you will provide physical, emotional, and spiritual support to patients and families when it's needed most.
Responsibilities- Conduct psychosocial evaluations to assess patients' and families' needs
- Provide ongoing counsel and support to patients and families
- Offer bereavement services to support patients and families through the grieving process
- Develop and implement community education programs to promote awareness and understanding of pediatric hospice care
- MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
- Current state license as a social worker
- 2+ years of clinical work experience, preferably in healthcare or hospice
- Reliable transportation
- Ability to sit, stand, bend, move intermittently, and lift at least 25lbs
