About the Role

We are seeking a dedicated and compassionate Pediatric Social Worker to join our team at Agape Care Group. As a Pediatric Social Worker, you will play a vital role in providing emotional, spiritual, and practical support to patients and their families.

Key Responsibilities
  • Conduct psychosocial evaluations to assess the needs of patients and their families
  • Develop and implement individualized plans of care to address the physical, emotional, and spiritual needs of patients
  • Provide ongoing counseling and support to patients and their families
  • Collaborate with healthcare teams to ensure comprehensive care
  • Participate in bereavement services and community education outreach
Requirements
  • Master's degree in Social Work from an accredited school
  • Current state license as a social worker
  • 2+ years of clinical work experience, preferably in healthcare or hospice
  • Reliable transportation and ability to lift at least 25lbs
