About the Role

We are seeking a compassionate and skilled Nurse Practitioner to join our team at Abode Care Partners, a leading provider of integrated medical services. As a Nurse Practitioner, you will play a vital role in delivering high-quality patient care to seniors and individuals with complex needs in various settings.

Responsibilities
  • Provide Evidence-Based Medical Care: Plan, direct, and evaluate patient medical needs to ensure optimal health outcomes.
  • Examine and Treat Patients: Conduct thorough examinations and provide treatment for chronic and urgent episodic illnesses, minor injuries, and other health issues.
  • Order and Interpret Diagnostic Testing: Order laboratory and diagnostic testing as needed and interpret results to inform patient care.
  • Collaborate with Healthcare Professionals: Work with caregivers and other healthcare professionals to provide interdisciplinary care and maintain a comprehensive plan of care.
  • Provide Referrals: Refer patients to other healthcare practitioners when necessary to ensure comprehensive care.
  • Maintain Patient Records: Ensure patient medical records are up-to-date and accurate.
Qualifications
  • Active State License: Hold an active state Nurse Practitioners license.
  • National Board Certification: Possess current Nurse Practitioner National Board Certification.
  • Active DEA Licensure: Hold active state DEA licensure.
  • Master's Degree in Nursing: Possess a Master's degree in Nursing (MSN).
  • Nursing Experience: Have five or more years of nursing experience, with at least two years of nurse practitioner experience preferred.
  • Interpersonal and Communication Skills: Possess strong interpersonal and communication skills, both verbal and written.
  • Time Management and Accountability: Demonstrate excellent time management skills and accountability for daily workflow.
  • Medical Instrument Proficiency: Possess proficiency in medical instruments and equipment.
  • Knowledge of HIPAA and OSHA: Have a strong knowledge base of HIPAA and OSHA regulations.
  • Computer-Based Data Management: Demonstrate proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications.
  • Licensed Driver: Possess a valid driver's license and automobile insurance.
  • Physical Ability: Be able to engage in moderate physical activity, such as handling objects up to 25 lbs., standing or walking for more than four hours per day, etc.
