Housecalls Nurse Practitioner

We are seeking a highly skilled Nurse Practitioner to join our Housecalls team at UnitedHealth Group. As a key member of our team, you will provide comprehensive assessments and care to patients in the comfort of their homes.
Key Responsibilities- Conduct thorough assessments to identify and address patients' physical, mental, and social needs.
- Effectively manage acute and chronic medical and behavioral conditions, providing evidence-based practice screenings and interventions.
- Communicate and collaborate with the interdisciplinary care team to ensure seamless care coordination.
- Provide patients and caregivers with education and counseling on disease processes, medication, and compliance.
- Attend training and ongoing education to stay up-to-date on the latest medical advancements and best practices.
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- Active and unrestricted license in the state where you reside, or ability to obtain by start date.
- Active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
- Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
- 1 year of clinical experience in a practice setting, preferably in long-term care or home health.
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
- Understanding of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end-of-life discussions.
