About the Role

We are seeking a highly skilled Nurse Practitioner to join our Housecalls team at UnitedHealth Group. As a key member of our team, you will provide comprehensive assessments and care to patients in the comfort of their homes.

Key Responsibilities
  • Conduct thorough assessments to identify and address patients' physical, mental, and social needs.
  • Effectively manage acute and chronic medical and behavioral conditions, providing evidence-based practice screenings and interventions.
  • Communicate and collaborate with the interdisciplinary care team to ensure seamless care coordination.
  • Provide patients and caregivers with education and counseling on disease processes, medication, and compliance.
  • Attend training and ongoing education to stay up-to-date on the latest medical advancements and best practices.
Requirements
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • Active and unrestricted license in the state where you reside, or ability to obtain by start date.
  • Active Nurse Practitioner certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center.
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
Preferred Qualifications
  • 1 year of clinical experience in a practice setting, preferably in long-term care or home health.
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end-of-life discussions.
