Private Duty Nurse - LVN/LPN
Aveanna Healthcare is a leading national provider of home health services to medically fragile children and adults. We are seeking a skilled and compassionate Private Duty Nurse - LVN/LPN to join our team and provide high-quality care to our patients in their homes.
Responsibilities:
- Implement and update the nursing care plan in collaboration with the patient's physician and healthcare team.
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Document all patient interactions, assessments, and care provided in a timely and accurate manner.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
- Current, unrestricted state license as a Licensed Nurse (LPN/LVN) in the state of practice.
- Current CPR certification from American Red Cross, American Heart Association, or BLS.
- One (1) year of experience as a Licensed Nurse (LPN/LVN) in a clinical care setting.
- Ability to work independently and as part of a team to provide high-quality patient care.
