Private Duty Nurse LVN-Full Time/Part Time Days

Aveanna Healthcare is seeking a skilled and compassionate Private Duty Nurse LVN to provide high-quality nursing care to our patients in their homes. As a Private Duty Nurse, you will work one-on-one with patients, providing personalized care and support to help them achieve their goals and improve their quality of life.
Responsibilities:- Implement and update the nursing care plan to meet the individual needs of each patient
- Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
- Assess and document patient progress, identifying areas for improvement and developing strategies to address them
- Collaborate with the healthcare team to ensure seamless care coordination and communication
- Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- One year of experience as a Licensed Nurse in a clinical care setting
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
