Key Responsibilities
  • Develop and implement a population health strategic plan that aligns with our organization's goals and objectives
  • Lead the development of evidence-based care management programs to improve health outcomes for our members
  • Collaborate with cross-functional teams to identify and address health disparities and social determinants of health
  • Utilize data and analytics to inform decision-making and drive population health initiatives
  • Develop and maintain relationships with key stakeholders, including healthcare providers, payers, and community organizations
Requirements
  • Bachelor's degree in Nursing or related field
  • Minimum of 10 years of clinical experience, with at least 5 years in a managed care organization
  • Experience in Texas Medicaid programs, specifically STAR Kids and STAR+PLUS
  • Proven leadership and management skills, with experience in strategic planning and team development
  • Advanced analytical and problem-solving skills, with ability to interpret statistical data and produce high-quality reports
