Key Responsibilities- Develop and implement a population health strategic plan that aligns with our organization's goals and objectives
- Lead the development of evidence-based care management programs to improve health outcomes for our members
- Collaborate with cross-functional teams to identify and address health disparities and social determinants of health
- Utilize data and analytics to inform decision-making and drive population health initiatives
- Develop and maintain relationships with key stakeholders, including healthcare providers, payers, and community organizations
- Bachelor's degree in Nursing or related field
- Minimum of 10 years of clinical experience, with at least 5 years in a managed care organization
- Experience in Texas Medicaid programs, specifically STAR Kids and STAR+PLUS
- Proven leadership and management skills, with experience in strategic planning and team development
- Advanced analytical and problem-solving skills, with ability to interpret statistical data and produce high-quality reports
