Nurse Consultant

The Defense Health Agency is seeking a highly skilled Nurse Consultant to join our team. As a key member of our population health management team, you will be responsible for planning, designing, implementing, and evaluating population health and disease management programs.
Key Responsibilities:
- Collect, review, and interpret population-based data to identify areas for improvement and implement evidence-based solutions.
- Collaborate with healthcare professionals to coordinate and ensure consistent health teaching, counseling, and guidance to individuals and families participating in disease management programs.
- Develop and implement population health improvement plans and HEDIS measures to support improved outcomes.
- Provide population health data summary reports to clinical primary care managers to identify high-risk populations.
- Develop and deliver training and program direction to clinical professionals and non-professionals on demand, disease, and condition management activities and interventions.
Requirements:
- Current, active, valid, and unrestricted nursing licensure in any state or province of the United States.
- Current Basic Life Support (BLS) certification.
- Advanced Cardiac Life Support (ACLS) certification or ability to pass within 6 months of employment.
- Pediatric Advanced Life Support (PALS) certification or ability to pass within 6 months of employment.
This is a Testing Designated Position (TDP) for drugs. Position requires State Criminal History Repository Check. Required to work rotate shifts, including weekends and holidays. Immunizations required. Position requires pre-employment physical due to heavy lifting.
